Hours:                   37.5 hours per week

Position Description: Are you interested in making a difference in the lives of elders?  Do you prefer to work within a community? Does gathering information, creating plans and putting them into action excite you?  If so, then our Case Manager position is right for you! We are looking for a Case Manager to be part of a creative team to visit our consumers in the home for initial and reassessments, develop and oversee individual consumer care plans and be their advocate.  Your work schedule will be a harmonious balance between visiting consumers in their homes and being in the office to create and produce care plans.  Sometimes these tasks will be routine like taking phone calls from consumers, working with vendor agencies to set up services, and making your appointments.  However, there will also be times for strategic thinking and certain challenges that will require you to think quickly on your feet to make good decisions that will have a profound impact on consumers' lives.  You will have one-on-one supervision to discuss your cases, participate in agency wide training and take part in a Case Manager Discussion group.  If this sounds like you, great! Qualifications:

  • Bachelor's degree required with social service experience.
  • Strong interviewing and assessment skills.
  • Excellent communication and documentation skills.
  • Truly comfortable using a computer.
  • Sensitivity to the needs of seniors and individuals with disabilities.
  • Valid driver's license and reliable vehicle.
  • Bilingual a plus.
